Consent Form - General Information and Conditions

A parent/guardian must be present to collect under 18’s at the end of the session, unless prearranged with us that the participant
will leave alone or by other means. If a parent/guardian is not present or hasn’t contacted us to advise of their late arrival
and/or can’t be contacted by us, we will have to contact the police.
- All participants must be able to swim 50 metres in open water. Please let us know if not confident.

As a responsible organisation, we will do our utmost to prevent any injuries but some will inevitably occur. The responsibility for
safety is shared between our administrators, coaches, trainers and you. The declaration below is intended to bring to your attention
the potential for physical injury and harm and your responsibility for preventing it. It does not absolve Weymouth Watersports and Fitness Club
from its responsibilities for your health and safety. Physical injuries include bumps and bruises, muscle strains, soft tissue injuries
and in exceptional circumstances, bone breaks, which may include serious spinal or head injuries. In very rare cases, deaths have
occurred whilst participating in water sports and training.

If you suffer from any medical condition(s) that may affect safety, e.g. asthma, epilepsy, diabetes, heart problems, back
and/or joint injuries, please give details below. This information will be treated in strict confidence.
DECLARATION
I have read and understood the above General Information and Conditions of Weymouth Watersports Ltd. In
addition its servants, agents and employees are not under any liability whatsoever in respect of loss or damage of
personal property, however caused, while using our services.
